Terms

• By sending this form I automatically authorize the school’s cafeteria to serve snack+lunch or lunch starting on the date the form is sent.
• After registering my son/daughter I will accept a voluntary commitment which implies a minimum permanency of one (1) month, clarifying and understanding that even if I cancel the service, payment will be mandatory for the rest of the month in which the withdrawal/cancellation occurs.
• Likewise I declare that I have full knowledge of the present contract and it can only be cancelled for students from 6th – 12th grade under the following causes/grounds:
  1. Under my request as long as it is notified 10 days prior to the end of the month that’s taking place.
  2. Pending debt for more than three months.
